1. How would you describe your current relationship with Jesus Christ?
   I’m still exploring what Christianity is about.
   I believe in God but haven’t made a personal commitment yet.
   I’ve committed to Jesus and I’m growing.
   I walk closely with Jesus and help others grow.
   I teach or mentor others regularly.
2. How confident are you in hearing God’s voice and recognizing His guidance?
   I’m not sure I’ve ever heard God speak to me.
   Sometimes I sense Him but doubt if it’s really Him.
   I often hear and obey His promptings.
   I help others learn to hear and follow His voice.
3. How consistent is your personal Scripture reading and prayer?
   Rarely.
   Occasionally.
   Several times a week.
   Daily and meaningful.
4. Are there recurring wounds, habits, or strongholds that keep you from walking freely with God?
   Yes, I feel stuck and need help.
   I’m working through some issues but still need more healing.
   I’ve experienced freedom and walk in it most of the time.
5. Have repentance and forgiveness brought clear freedom in your life?
   Not yet.
   I’ve taken some steps but still feel burdened.
   Yes, I’ve practiced this and feel free.
6. How clear are you about your spiritual gifts and life calling?
   I’m unsure what my gifts or calling are.
   I have some sense but it’s not very clear.
   I know my gifts and where God is calling me to serve.
7. Have you used your spiritual gifts to serve others?
   Not yet.

   Yes, regularly.
8. Are you currently discipling or mentoring anyone in their faith?
   No, I’m not doing this yet.
   Yes, informally.

9. How often do you share your testimony or faith story?
   I haven’t really shared it.
   I’ve shared once or twice.
   I share it frequently, including online or in public settings.
10. Are you currently leading or creating content that equips others (e.g. groups, teaching, podcast, videos)?
    Not yet.
    I’d like to but need help starting.
    Yes, I actively lead or create content to equip others.
